Bug 3791: Add gateway MAC to Prefix flowmod match.

The current implementation of flow-mods for
Layer 3 IP prefixes doesn't provide a gateway
MAC address for the matcher. This patch creates
separate flow-mods for each subnet with a gateway
on the node for this prefix.
